Before Your Appointment:

There are a few key steps to take before your first dental visit. First, we advise you to gather any relevant paperwork and information including the following:

YOUR CURRENT MEDICATION LIST
DENTAL INSURANCE INFORMATION
NOTES FOR ANY RELEVANT MEDICAL CONDITION THAT MAY IMPACT TREATMENT
RELEVANT MEDICAL CONDITION THAT MAY IMPACT TREATMENT, INCLUDING ARTIFICIAL HEART VALVES/JOINTS, DIABETES, HIGH BLOOD PRESSURE, ETC.

Unparalleled Warranty

We believe in the quality of our dental work and want to pass on that confidence to you. Keep up on your dental routine 3, 4, or 6-month dental cleaning checkups and maintenance as needed and if something breaks, we have done, we’ll fix it.

Warranty Details:
Crowns that are on implants that FAIL will be replaced at cost if the failure is the consequence of the implant failure and NOT the prosthetics.

Warranty is only valid for active patients and applies to normal function. Warranty is void if the patient fails to appear within 30 days of their required appointment for periodic evaluation. For the warranty to remain valid, you must complete your treatment with our office and keep your subsequent maintenance appointments (every 3, 4, or 6 months), along with using any prescribed fluoride or bite guard treatments, all of which are subject to normal or discounted charges, during the same 5-year period. Exemptions: Act of war or Terrorism

While we operate as an out-of-network practice, we want to clarify that this choice doesn’t imply that your insurance won’t provide coverage. In fact, Dr. Hawk's treatments are covered under most PPO's. We are happy to complete the paperwork on your behalf, ensuring that you receive the maximum benefits you’re entitled to.

Any reimbursement will be sent to you directly.

Payment Policies

Hawk Dental Artistry is a fee-for-service practice. Payment is required in full at the time of your appointment.

We offer easy payment plans and financing options through care credit to fit your budget.

We accept all major credit cards, cash and checks.
